School Nurse

As the school year comes to an end, it’s a great time to go over paperwork needed for the start of the next school year!!  

If you are visiting your physicians over the summer, please reference the State Requirement schedule below to make sure you have the proper paperwork with you before going.  The HS office also has paper copies available for pickup.  All grade level paperwork will also be sent in summer mailings in August. 

If your child has a specific illness that requires a care plan a paper copy will be sent in the 4th quarter report cards. Those in need of copies before then or a newly diagnosed individual can print forms from this page– to the right. 

11th grade students can use their sports physical as an 11th grade physical – please provide nurse with extra copy

State Requirements for 7-12

( all BMI/Ht/Wt/Vision- performed by School Nurse)


Grade 7: 

  • Dental- Private exam or School Dental exam in November
  • Hearing Screening- performed by School Nurses (unless otherwise requested by parent)
  • Scoliosis Screening- performed by School Nurses (Females) and PT (males) or family physician (unless otherwise requested by parent)
  • Body Mass Index (BMI), Height, Weight & Vision. -performed by School Nurses (unless otherwise requested by parent)

*6th grade physical, Tdap and MVC4 vaccines MUST be completed before the start of the 7th grade school year. 


Grade 8: BMI, Height, Weight & Vision.(unless otherwise requested by parent
Grade 9: BMI, Height, Weight & Vision.(unless otherwise requested by parent)      Grade 10: BMI, Height, Weight & Vision. (unless otherwise requested by parent)

                                                                                                                                               Grade 11: 

  • BMI, Height, Weight & Vision-  (unless otherwise requested by parent)                                                                    
  • Hearing Screening- performed by School Nurses(unless otherwise requested by parent)
  •  Physical- private exam or school physician in April  (Sports physicals count for 11th grade physical- please provide a copy to school nurse)        
  •  2nd Meningitis booster

Grade 12: BMI, Height, Weight & Vision. (unless otherwise requested by parent)                                                                      Proof of 2nd Meningitis upon entry to 12th 

** Dental, Scoliosis, and Physical exams may be performed by your private physician, if desired. Students who choose to have screenings done by their physician have until November 1, 2023 to return the completed forms to the nurse’s office. If the appropriate forms are not received by that date, the student will be screened by the school’s dentist and/or physician (unless the nurse is otherwise notified).

Immunization Information(must provide proof prior to day 1 of new school year):

  1. Those Entering 7th grade:
    Meningitis (MCV4)
    TDAP
  2. Those Entering 12th grade  Meningitis #2 (MCV4) (Booster usually given at 16 yr old physical in 11th)
